- Purchase Request: Complete the form, have it signed by Faculty, and submit to staff in Sloan 201. The office staff will then prepare and process all purchasing documents. If you are going to pickup the item, return to Sloan 201 for a university purchase order to take with you to pickup the item(s).
- Travel Request: Complete the form and submit to staff in Sloan 201. The office staff will then inform you how to proceed to arrange for your travel.
